Club sponsors please collect this form from each club participant.  Keep a copy on file and submit copies to Maryann Soley, Assistant Principal (521), and Cristina Pacione-Zayas, CoC Coordinator (625).

Club sponsors please use this meeting sign-in sheet for each session you meet with participants.

Use this application to apply for CoC funds and/or inform administration of your intent to start an after school club.

Club sponsors use this document to keep running attendance of your club participatns.
